Here is the Polling Place Accessibility Checklist PDF (PPAC_1.3.pdf) that Ben Behrens demonstrated at the Polling Place Survey training in Fresno.

Attached to the checklist is a Read Me file & an Excel template file.

Please keep in mind that this is the 2010 version NOT the new 2014 version.

When the 2014 version is completed, it will be posted here.

Instructions: 

The above survey tool represents the SOS 2010 checklist for polling place accessibility. The PDF would be loaded on a Windows based tablet or smart phone that is equipped with Acrobat Reader. The checklist is dynamic and has menus to allow you to navigate through the sections. When you complete it, you can email it to the office or save it to the device. The data can be exported into Excel (as shown) so it can be uploaded to your Election Information Management System.
